If it stem is turning black it's melting, melted parts as far as I know are not salvageable. What I normally do is trim the stem to where it is not melted and replant it. Melt from what I've seen in my tank travels upward until your plant eventually dies. Try not to plant stem plants in a bundle but instead plant them a little spaced apart. Check your water temperature to make sure it is not too warm for the plant. If you have a side shoot, try to plant that under the gravel along with the stem. I think the shoots become roots? But I cannot confirm this, perhaps another member of the forum knows. Once the plant roots it usually becomes more stable. If a some leaves fall off that's normal shock you'd expect but if the stem starts to go it's my opinion it would be best to trim the melt and try again. Another thing you can try is floating the plant if it is in really bad shape. Good luck.
