When to re-dirt?

So, I have a 90 gallon dirted with flourite on top (no co2, regular water changes). Been going about two years. Heres the current problems: cryps and amazon swords are getting holes in the leaves, amazon swords are pushing themselves out of the gravel(rhizomes are now completely out) and the roots are also poking out of the substraight; also swords are growing curled leaves.
